About The Position

Sholom is a mission-driven nonprofit senior care provider in the Twin Cities, offering services across independent living, assisted living, memory care, hospice, home care, and more. The role supports residents and families through advocacy, ongoing assessment and evaluation, service coordination, and care planning. Its primary focus is residents’ quality of life and helping them achieve optimal psychosocial, emotional, and behavioral well-being. The position covers the Transitional Care Unit and reports to the Campus Director of Social Services. The posted salary range is $57,000–$67,000 per year. A $2,000 sign-on bonus is paid over the first year of employment. The schedule is 8:00 a.m.–4:30 p.m., Monday through Friday.

Requirements

Bachelor’s degree in Social Work, Gerontology, or a related field; an equivalent combination of professional experience and education may be considered at management’s discretion.
Licensed by the Minnesota Board of Social Work and maintain unrestricted licensure.

Nice To Haves

One or more years of social work experience, preferably in a long-term care environment.
For the Transitional Care Unit, one or more years of social work experience in a long-term care setting is preferred.

Responsibilities

Implement resident services consistent with regulations and established best practices.
Provide resident and family advocacy.
Conduct ongoing assessment and evaluation.
Coordinate services and care planning.
Support residents’ quality of life and psychosocial, emotional, and behavioral well-being.
Cover the Transitional Care Unit.
