About The Position

This role involves orchestrating the overall plan of care from admission to discharge, integrating patient and family preferences into a cohesive strategy across the entire continuum of care. The Case Manager Social Worker will serve as the clinical expert in assessing and treating complex needs, including legal issues, neglect, substance abuse, crisis intervention, and psychosocial adjustment. They will proactively identify and dismantle barriers to care, advocating for the internal and community resources necessary to ensure patient safety and health equity. A continuous dialogue with the care transition team is maintained to reevaluate health plans and provide expert guidance on medical record documentation, complex level-of-care recommendations, and post-discharge options.

Requirements

High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
Acute care case management experience
MSW

Nice To Haves

Clinical Counselor Preferred.
Social Worker specializing in Clinical Practice credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.
