Case Manager/Social Worker
About The Position
Lutheran Family and Children’s Services (LFCS) is hiring a Case Manager/Social Worker to help children and families overcome challenges and build safety and stability. The role coordinates and provides client-centered care and offers case management, pregnancy counseling, parenting and adoption education, community education and outreach, and post-adoption support for birth parents. The position works with clients to create service plans based on their goals, then provides counseling, advocacy, and instruction in person at clients’ homes or virtually. It also responds to client crises according to program parameters and protocols.
Requirements
- Bachelor's or Master's degree in Social Work, Counseling, Psychology, Criminal Justice, or another human behavior science.
- Licensed driver with access to dependable personal transportation and auto insurance.
- Passion for making a difference and supporting families through life’s challenges.
Nice To Haves
- Two years of experience in child welfare, working with children and families.
Responsibilities
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
- Provide case management, pregnancy counseling, parenting and adoption education, community education and outreach, and post-adoption support for birth parents.
- Develop service plans with clients, including goals and objectives aligned with what clients wish to accomplish.
- Counsel, advocate for, and teach clients skills identified in their service plans, face to face in the home or virtually.
- Respond to client crisis situations within program parameters and in a timely manner according to program protocols.
