About The Position

Under the direction of the Director, Case Management, the Case Manager (RN) collaborates with patients, patient family members, physicians, and other healthcare professionals to design, implement, and evaluate plans of care specific to each patient's needs. The position supports high-quality, cost-effective patient care in the Emergency Room.

Requirements

• Meet education requirements for RN licensure.
• Hold a valid, current RN state license or compact license.
• Have at least three years of previous clinical and/or case management/utilization management experience, preferably in a high-volume acute care hospital or healthcare facility.
• Demonstrate excellent interpersonal, communication, and problem-solving skills to gather and exchange data with medical professionals, patients, and patient family members.
• Effectively use critical-thinking and problem-resolution skills.
• Effectively communicate with community stakeholders to assist patients.

Nice To Haves

• Bachelor's degree preferred.
