About The Position

The Recovery and Reentry Case Manager manages an assigned caseload of justice-involved adults with substance use disorders and co-occurring behavioral health conditions who are incarcerated within the Blue Ridge Regional Jail Authority and transitioning back into the community. The incumbent is responsible for conducting comprehensive needs assessments, developing individualized reentry and service plans, coordinating evidence-based treatment and recovery supports, and ensuring continuity of care from incarceration through community reintegration. The Recovery and Reentry Case Manager collaborates closely with correction staff, behavioral health providers, peer recovery specialist, probation and parole, healthcare providers, housing agencies, employers, and other community partners to reduce overdose risk, promote treatment, and coordinating medication-assisted treatment (MAT/MUD) continuity, addressing barriers related to housing, employment, transportation, benefits, and healthcare, monitoring participant progress, and maintaining timely, accurate, documentation in accordance with agency, funding, and regulatory requirements. This is a grant-funded position that may terminate with the ending of the grant funding.

Requirements

Bachelor’s degree from an accredited college or university in an applicable human services field; or relevant education which indicates that the individual possesses the required knowledge, skills, and abilities.
A minimum of one (1) year of relevant mental health, developmental disability, and/or substance use work experience with the identified population, or a combination thereof that demonstrates that the candidate possesses the required knowledge, skills, and abilities; and/or an equivalent combination of education, training, and experience that provides the individual possesses the required knowledge, skills, and abilities.
Considerable knowledge of substance use disorders and DSM 5 disorders of adulthood.
Knowledge of services, systems, and programs available in the community including primary health care, support services, eligibility criteria and intake processes, generic community resources, and mental health, developmental disabilities, and substance abuse treatment programs.
Knowledge of the nature of serious mental illness, developmental disabilities, and substance abuse depending on the population served, including clinical and developmental issues.
Knowledge of different types of assessments, including functional assessments, and their uses in service planning.
Knowledge of treatment modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination.
Knowledge of the service planning process and major components of a service plan.
Knowledge of the use of medications in the care or treatment of the population served.
Knowledge of all applicable federal and state laws, state regulations, and local ordinances.
Ability to identify and document an individual's needs for resources, services and other supports.
Ability to use information from assessments, evaluations, observation, and interviews to develop individual service plans.
Ability to identify services and resources within the community and established service system to meet the individual's needs and document how resources, services, and natural supports, such as family, can be utilized to achieve an individual's personal habilitative/rehabilitative and life goals.
Ability to coordinate the provision of services by public and private providers.
Ability to work as team members, maintaining effective inter- and intra-agency relationships.
Ability to work independently, performing position duties under general supervision.
Ability to engage and sustain ongoing relationships with individuals receiving services.
Must possess a valid Virginia driver’s license.
Willingness to travel to any Horizon worksite, transport clients, and provide services to clients at other locations.
Willingness to travel to a variety of Horizon sites for regular attendance at trainings and Horizon meetings.
