Central Plains Center is seeking a dedicated individual to join their family as a Case Manager / QMHP. This role focuses on improving the quality of life for persons with mental illness, Intellectual Disabilities, and chemical dependency, and their families, by providing accessible services and resources that support individual choices and promote lives of dignity and independence. The position involves providing curriculum-based skills training and service coordination activities for children and adolescents eligible for services, developing and maintaining curriculum for skills training, and providing service coordination including linking clients to appropriate services and advocating for them. The role also includes providing skills training for consumers and their family members, ensuring proper billing practices, maintaining regular contact with individuals served to monitor services and identify needs, and providing crisis intervention services to individuals with mental illness.
