About The Position

We’re seeking a dynamic and innovative QMHP/T for expanding Mental Health Skill Building and Intensive In-Home programs in Hampton, Virginia. The role provides Intensive In-Home therapy for children and Mental Health Skill Building for adults to help them achieve a better life and make a lasting impact on the mental health community.

Requirements

• Bachelor’s degree in Psychology, Sociology, Human Services, or a related field.
• Experience working with children.
• Must qualify as a Qualified Mental Health Professional (QMHP) or be in training through the Virginia Department of Health Professions.

Responsibilities

• Develop and maintain Individual Service Plans and progress notes.
• Work with individuals to foster new or improved skills.
• Perform case management functions, including linking, arranging, and advocating for services and referrals.
• Maintain up-to-date individual records and prepare required session notes and reports in a timely and accurate manner.
• Establish and maintain good working relationships with public and private agencies and services, and collaborate with them.
• Conduct ongoing assessments of individuals’ treatment needs.
• Maintain continuous contact with identified individuals and other key stakeholders in planning and service delivery.
• Maintain at least two Intensive In-Home cases for children.
• Maintain at least four cases total.
