Case Manager Licensed Professional (RN) Hospice FT
About The Position
The Case Manager Licensed Professional (RN) Hospice provides coordination of care, continuously assesses the needs of patients and families, and implements the interdisciplinary care plan. The position performs services in accordance with physician orders and the established plan of care, ensuring care is delivered in compliance with federal and state regulations. The role requires a current license in the state of practice, at least one year of experience as a licensed professional, basic technology skills, and CPR certification for healthcare providers. A valid driver license, automobile liability insurance, dependable transportation, and the ability to drive safely in all weather conditions are also required.
Requirements
- Graduate of an approved school of clinical education.
- Licensed in the state where currently practicing.
- At least one year of experience as a licensed professional.
- Basic demonstrated technology skills, including operating a mobile device.
- Possess a valid state driver license.
- Maintain automobile liability insurance as required by law.
- Maintain dependable transportation in good working condition.
- Be able to safely drive an automobile in all types of weather conditions.
- Possess CPR certification for the healthcare provider.
Nice To Haves
- Medicare home health or hospice experience is preferred.
- Experience as a licensed professional may be deferred with review and approval from regional and executive service line clinical leadership.
