About The Position

Enhabit Home Health & Hospice is seeking a licensed professional to coordinate hospice care. The role involves assessing the ongoing needs of patients and their families and implementing the interdisciplinary plan of care. Services must be provided in accordance with physician orders, the established plan of care, and applicable federal and state regulations.

Requirements

• Graduate of an approved school of clinical education.
• Licensed in the state where currently practicing.
• At least one year of experience as a licensed professional.
• Basic demonstrated technology skills, including operation of a mobile device.
• Valid state driver's license.
• Maintain automobile liability insurance as required by law.
• Maintain dependable transportation in good working condition.
• Ability to safely drive an automobile in all types of weather conditions.
• CPR certification for the healthcare provider.

Nice To Haves

• Medicare home health or hospice experience is preferred.
• Experience as a licensed professional may be deferred with review and approval from regional and executive service line clinical leadership.

Responsibilities

• Coordinate care for patients and their families.
• Ensure continuous assessment of each patient's and family's needs.
• Implement the interdisciplinary care plan.
• Perform services in accordance with physician orders and the established plan of care.
• Ensure delivery of care complies with federal and state regulations.
