About The Position

New Day Personal Care Services is seeking a Case Manager in Training to support the coordination and delivery of quality in-home care services. This entry-level position is designed for individuals interested in developing the skills and experience necessary to become a successful Case Manager. The Case Manager in Training will work closely with experienced team members while learning client coordination, caregiver management, scheduling, documentation, service monitoring, and agency procedures. This position requires strong communication, organization, professionalism, and the ability to respond effectively to changing client and staffing needs.

Requirements

  • High school diploma or equivalent required.
  • Strong verbal and written communication skills.
  • Strong organizational and time-management skills.
  • Ability to prioritize multiple responsibilities and meet deadlines.
  • Basic computer proficiency and ability to learn electronic documentation systems.
  • Ability to maintain professionalism and confidentiality.
  • Strong problem-solving skills and attention to detail.
  • Ability to work independently and collaboratively as part of a team.
  • Must be dependable and able to participate in the on-call rotation.
  • Valid driver's license and reliable transportation required.

Nice To Haves

  • Previous experience in caregiving, healthcare, human services, customer service, or an administrative environment preferred.
  • Previous case management experience is not required.
  • Previous experience as a Direct Support Worker (DSW), caregiver, CNA, or in a related direct-care position.
  • Experience working with elderly individuals or individuals with disabilities.
  • Experience communicating with clients, families, or healthcare professionals.
  • Interest in developing leadership, case management, and supervisory skills.

Responsibilities

  • Assist with coordinating services for clients within an assigned service area.
  • Communicate with clients, responsible representatives, caregivers, and internal staff regarding services and scheduling.
  • Assist with caregiver staffing, scheduling, call-outs, and coverage needs.
  • Maintain accurate and timely client and caregiver documentation.
  • Monitor caregiver attendance, service delivery, and required documentation.
  • Assist with addressing client and caregiver concerns and escalating issues when appropriate.
  • Conduct client and caregiver follow-up as assigned.
  • Assist with maintaining compliance with agency policies, procedures, and applicable program requirements.
  • Participate in staff meetings, training, coaching, and professional development.
  • Learn and utilize agency software, electronic documentation, and communication systems.
  • Travel throughout the assigned service area as necessary.
  • Participate in a rotating on-call schedule to assist with after-hours client and staffing needs.
  • Perform additional duties as assigned.
