About The Position

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. The Justice Involved Mental Health FSP serves adults with serious mental illness and histories of criminal justice involvement. This outpatient, ACT treatment model includes wrap-around services and supports with 80% of services provided in the community. Our JIMH FSP supports clients in meeting basic needs (e.g., food, clothing, shelter) and obtaining benefits. We also work to educate and support them as they navigate additional community resources that increase their self-sufficiency, minimizing the possibility that they may turn to criminal acts to get these needs met.

Requirements

  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
  • Valid and current driver’s license
  • Personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
