About The Position

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. This is a full-time, benefits-eligible position through June 30, 2027, with potential opportunities for transfer or continuing employment based on operational needs and performance. Telecare's mission is to deliver excellent and effective behavioral health services that engage individuals in recovering their health, hopes, and dreams. In partnership with San Diego Behavioral Health Services and San Diego Housing Commission, Telecare offers onsite supportive and case management services for 75 persons residing at The Shores and Pacific Village. Service participation is voluntary and not a requirement of residing at The Shores and Pacific Village. Telecare works with tenants to engage them in services according to their behavioral health and case management needs. Services are provided throughout the community based on preference: on-site in The Shores and Pacific Village PSH community space; in individual units; in the supportive service office; or in the community, if appropriate. These services focus on housing stability and retention.

Requirements

  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
  • Valid and current driver’s license
  • Personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
  • Willingness to use your personal vehicle to attend meetings, etc.

Responsibilities

  • Attends meetings.
