Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. The Kern Youth Family Resource Center is an innovative, family-friendly program designed to provide youth and families with timely support when they need it most. In a welcoming, trauma-informed environment, the FRC connects families with resources, support, and services tailored to their individual needs. As part of Kern County’s broader continuum of care, the program works collaboratively with families, community partners, and existing providers to help youth successfully navigate challenges and connect with ongoing support.
