People Incorporated is hiring a Case Manager I or II for its Targeted Case Management team serving Dakota County, Minnesota. The role primarily supports clients diagnosed with chronic mental illness or other chronic health conditions. Case Managers connect clients with services by developing person-centered support plans that help them live as independently as possible. They build long-term relationships with clients, work autonomously, and create their own schedules. The position is full-time, Monday through Friday during business hours, and hybrid, with work based in Dakota County. The Case Manager conducts functional assessments, develops and reviews individual plans, coordinates access to mental health and related services, assesses client needs, and makes referrals. The role also involves monitoring service effectiveness and continuity of care, maintaining records and funding-agency reports, documenting interventions and client interactions according to professional and organizational standards, and attending monthly client meetings. Applicants must meet the applicable education and experience requirements for Case Manager I or II, have a current valid driver’s license and auto insurance, and pass required background and federal Health and Human Services Office of Inspector General screening. A $1,000 hiring bonus is offered to external candidates. Compensation depends on qualifications and is listed separately for each level.
