About The Position

The Fatherhood Case Manager provides individual coaching and personalized support to fathers through the Fatherhood Project. Dedicated coaches offer guidance with goal setting, parenting support, resource coordination, referrals, and case management. They are trained to facilitate the Nurturing Fathers Program.

Requirements

Associate’s degree in behavioral science or related field, or equivalent work experience as allowed/required by regulatory bodies.
Driver License and valid auto insurance.
Ability to learn case management principles, healthcare management, and reimbursement requirements.
Excellent organizational and time management skills.
Effective verbal and written communication skills.
Ability to relate to and interact with a wide variety of people.
Advanced level problem-solving skills and ability to multi-task while prioritizing.
Ability to utilize technology including electronic service documentation systems.
Basic computer skills with ability to navigate Microsoft applications including Word, Outlook, Excel, Teams, and PowerPoint.

Responsibilities

Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered under minimal supervision.
Maintain caseload and utilization by monitoring and assessing progress through regular contact.
Handle case assignments, draft service plans, review case progress, and determine case closure for review and/or in consultation with a supervisor.
Deliver ongoing services including crisis intervention, advocacy, medication monitoring, transportation assistance, financial budgeting, housing assistance, and referral/linkage to community resources.
Assist clients in achieving wellness and autonomy through supportive counseling and care coordination.
Develop and/or implement individualized treatment or service plans and ensure compliance with standards and agency policies.
Document all clinical activities accurately and timely to meet program, funder, and credentialing entity criteria.
Participate in treatment teams and assist in making recommendations for status changes.
May assist in providing education and/or group curriculum.
Adhere to professional standards as outlined by protocols, rules, and regulations.
Complete agency, accrediting body, and funder required training.
Other duties as assigned.
