About The Position

Ruby's Place dba Restorative Pathways (RP) is a nonprofit providing no-cost, low-barrier, trauma-sensitive, client-centered services to people affected by domestic violence, human trafficking, and violent crime. The Clinical Case Manager provides integrated case management and clinical support to adults and families experiencing homelessness, including adults recovering from illness, injury, or hospitalization. The role bridges medical care, behavioral health, and housing to help clients stabilize physically and emotionally, follow care plans, and move toward permanent housing. Services are trauma-informed, strengths-based, culturally responsive, and harm reduction-oriented. The position includes psychosocial assessment, service planning, short-term therapeutic intervention, crisis response, and coordination of community resources. Care planning may address basic needs such as housing, food, income or benefits, transportation, and healthcare access, as well as medical or psychiatric discharge instructions, medication, wound care, follow-up appointments, and transportation. Coordination with medical providers requires signed releases. The role also connects clients with behavioral health and substance use services, using harm reduction where appropriate. The position is full-time, 40 hours per week, non-exempt, and based in Tracy, California. Although the posting initially describes the role as in-person, the working-conditions section states that it is eligible for hybrid in-person and remote work; on-site and remote time will be discussed with the supervisor, and on-site availability is required as business needs dictate. Local travel in the greater Bay Area may be required. The position receives regular clinical supervision from a licensed clinician or clinical supervisor, including weekly individual supervision and weekly group supervision or case conferences.
