About The Position

Community Homes for Opportunity (CHO) assists individuals over the age of 18 living with serious mental illness by providing stable, affordable, appropriate housing and support services. CHO aims to support individuals to achieve and maintain physical, mental health, and wellbeing, as well as foster independence and enhance participation and integration into the community of their choice. CHO is a person-centered, flexible service, reflecting the individual and their changing needs. The Housing Case Manager will collaborate with home operators and tenants of four (4) congregate living homes to support the best possible living environments. They will work to support referrals to congregate living homes, support home operators with operations, and facilitate tenant access to resources and supports to maximize their independence and potential and promote their recovery.

Requirements

  • A Bachelor’s degree in behavioral science or equivalent (equivalency for the purpose of qualification means a university education in the social sciences, which includes or is supplemented by practical courses in these disciplines).
  • Experience providing case management services to those experiencing serious mental illness/addictions.
  • Familiarity with Psychosocial Rehabilitation principles and practices and the concept of
  • An understanding of standards that apply to congregate living environments, the housing rental market, homelessness, and poverty issues.
  • Working knowledge of the Residential Tenancy Act (RTA).
  • A commitment to the rights of those experiencing serious mental illness/addictions.
  • Knowledge and awareness of Indigenous culture and language considered an asset.
  • Ability to work autonomously as well as cooperatively within a team environment in a rural setting.
  • Excellent communication, organization, and interpersonal skills.
  • Working knowledge of Microsoft Windows, the Internet, and data management systems.
  • A willingness to serve clients within the communities of the District of Muskoka and Parry Sound.
  • A valid Ontario G or G2 driver’s license, a safe and reliable means of transportation, and appropriate insurance for work purposes on your vehicle with a minimum of $1,000,000 in liability coverage.

  • Registration, membership, or application for membership in the college appropriate to your discipline is an asset.
  • Bilingual in French/English language considered an asset.
