About The Position

The Community Outreach and Support Team (COAST) provides intensive case management and assertive community treatment for people with serious mental illness. The Case Manager with the COAST team is responsible for carrying out case management, psychosocial rehabilitation, crisis intervention and treatment for individuals living with mental illness. This team member has clinical skills and experiences to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services with clients with severe and persistent mental illness under the clinical supervision of the team leader.

Requirements

  • Bachelor’s degree in a human service/health science discipline relevant to the position. Equivalency may be considered for a combination of educational experiences.
  • Mental Health related diploma with relevant work experience PLUS registration with a regulated health professional college
  • One year of experience working with persons diagnosed with mental illness and/or developmental disability and/or substance use issues preferred
  • Experience in providing supportive counselling
  • Experience in providing case management
  • Experience in providing crisis intervention
  • Ability to manage internal and external relationships
  • Possess and demonstrate empathy and compassion
  • Awareness of emotional intelligence
  • Conflict resolution skills
  • Understanding of the feelings, ideas and beliefs of others
  • Familiarity with the Residential Tenancy Act
  • Indigenous Cultural Awareness and Education is a requirement
  • Ability to communicate effectively, both oral and in writing
  • Tolerance of and ability to handle a heavy workload and competing demands and high risk situations
  • Ability to use and learn new technology (computers, software programs, phones, printer/scanners, alarm/security systems etc.)
  • Hold a valid Ontario G or G2 driver’s license
  • Have a reliable means of transportation
  • Appropriate insurance for work purposes on your vehicle with a minimum of $1,000,000 in liability coverage
  • Willingness to work days/evenings (including weekends and statutory holidays)

Nice To Haves

  • Bilingual in French is an asset.
