About The Position

The Case Manager is responsible for carrying out case management, psychosocial rehabilitation, crisis intervention and treatment for individuals living with mental illness. This team member has the clinical skills and experience necessary to assess, plan, develop, co-ordinate, and provide treatment by way of rehabilitative and supportive services in a multi-disciplinary environment. The Case Manager develops supports and opportunities for clients to increase or develop new skills needed to support success and satisfaction in their daily living, learning, working and social environments with the least amount of professional intervention required. This team member will be an expert in engaging the community and developing a network of landlords, community service providers and clients in the District of Muskoka.

Requirements

  • Bachelor’s degree in a human service/health science discipline relevant to the position, or equivalency may be considered for a combination of educational experiences (e.g., Mental Health related diploma with relevant work experience PLUS registration with a regulated health professional college).
  • One year of experience working with persons who have mental illness and/or developmental disability and/or substance use issues preferred.
  • Experience in providing supportive counselling, case management, or crisis intervention.
  • Ability to manage internal and external relationships.
  • Demonstrated empathy and compassion.
  • Awareness of emotional intelligence, conflict resolution, and understanding of the feelings, ideas, and beliefs of others.
  • Familiarity with the Residential Tenancy Act.
  • Ability to work autonomously as well as cooperatively within a team environment in a rural setting.
  • Excellent communication, organization, and interpersonal skills.
  • Working knowledge of Microsoft Windows, the Internet, and data management software.
  • A willingness to serve clients within the communities of the District of Muskoka and Parry Sound.
  • Must hold a valid Ontario G or G2 driver’s license, have a reliable means of transportation and appropriate insurance for work purposes on your vehicle with a minimum of $1,000,000 in liability coverage.
  • Willingness to work days/evenings (including weekends and statutory holidays).
