About The Position

The Case Manager 1 provides staff support and direct client care through comprehensive case management services, including intake assessments, development and implementation of individual care plans, and coordination with treatment providers and community resources. The role works within Adult Services on the Residential Behavioral Health Treatment care team, supporting clients’ recovery from mental illness, substance use, and homelessness. The position is non-exempt and works under the supervision of program management.

West Village provides individualized case management and life-skills services for adults with histories of chronic homelessness and mental health and/or substance use disorders. The program aims to help clients maintain housing and improve day-to-day functioning.

The role is based in New Haven and is full-time, 40 hours per week, Monday through Friday, 9:00 a.m. to 5:00 p.m. The listed salary is $22.00 per hour. Newly hired employees must attend a mandatory two-day orientation at the organization’s Roscommon office in Middletown.

Requirements

• Some post-secondary education.
• At least 2 years of related experience in direct service and case management.
• Valid Connecticut driver’s license.
• Regular and predictable attendance.
