Case Manager 1 - Eddy Shelter (Middletown)
About The Position
The Case Manager 1 provides staff support and direct client care at Eddy Shelter in Middletown, Connecticut. The role delivers comprehensive case management services, including intake assessments, development and implementation of individual care plans, and coordination with treatment providers and community resources. Working within Adult Services on the Residential Behavioral Health Treatment care team, the Case Manager supports clients’ recovery from mental illness, substance use, and homelessness. The position is non-exempt and works under the supervision of program management. Eddy Shelter provides emergency shelter, housing case management, and rapid rehousing services to homeless adults to maintain their safety while identifying long-term housing. The role is full-time, 40 hours per week, on the first shift, Monday through Friday from 10 a.m. to 6 p.m. Newly hired employees must attend a two-day mandatory New Hire Orientation at the Roscommon office in Middletown.
Requirements
- Some post-secondary education.
- At least 2 years of related experience in direct service and case management.
- Valid Connecticut driver’s license.
- Regular and predictable attendance is required.
