About The Position

The Centers is seeking a dedicated and experienced Care Management Lead for Behavioral Health Case Management to join our Gordon Square Clinical Site team. Reporting to the Clinical Site Director, this leadership role is at the heart of our mission: delivering whole-person, integrated care to individuals living with mental health, substance use, and co-occurring needs. You will lead a dynamic team of case managers while maintaining your own caseload — combining clinical excellence with situational leadership to drive meaningful outcomes for our clients and community. Requirements

  • Bachelor's or Master's degree in Social Work required
  • Minimum of three years of related work experience in a non-profit or health-related organization
  • LSW licensure, or license-eligible with commitment to obtain licensure within 12 months of accepting the position
  • Demonstrated experience in team leadership, clinical supervision, or care coordination

  • Experience in behavioral health, mental health, or substance use services strongly preferred

Responsibilities

  • Serve as the point person for Case Management Services alongside the Clinical Site Director, managing referrals and overseeing team performance.
  • Provide administrative supervision including monitoring progress notes, timekeeping, productivity measures, and case assignments.
  • Actively support the Site Director in recruiting, coaching, mentoring, and developing team members, including onboarding new staff.
  • Foster a spirit of teamwork, unity, and psychological safety that embraces diversity, resolves conflict constructively, and helps every employee succeed.
  • Provide individualized care coordination across healthcare, behavioral healthcare, and non-healthcare services, ensuring that each client's biopsychosocial needs are met holistically.
  • Perform therapeutic behavioral services, collateral contacts, and services delivered in all settings that meet the client's needs.
  • Build and maintain relationships with community partners who may refer clients to our services.
  • Provide direct support to clients in navigating The Centers' systems.
  • Actively link clients to health promotion, wellness activities, and community resources.
  • Complete clinical records and track billable hours.
  • Ensure the team's progress notes are completed within 72 hours of service.
