About The Position

Adult Services: Case Coordinators are responsible for providing comprehensive case management and support services to individuals with mental health and/or substance use disorders living in the community. This role involves conducting assessments, developing individualized service plans, coordinating care, and advocating for clients' access to community resources. The Case Coordinator will work collaboratively with clients, families, and community partners to promote recovery, independence, and overall well-being. Requirements

• Bachelor’s degree in social work, psychology, counseling, or a related field.
• Minimum of 2 years of experience in case management, or a related field.
• Valid driver’s license and reliable transportation for community-based work.

Nice To Haves

• Relevant licensure or certification preferred.
• CPR and First Aid certification preferred.
