About The Position

We are currently seeking a Case Coordinator to join our inpatient team. In this role, you will play a vital part in supporting patients’ transitions of care by coordinating aftercare services, maintaining communication with referral sources, and ensuring continuity throughout the treatment process.

Requirements

• High school diploma or equivalent required
• Experience in behavioral health, case management, or healthcare coordination preferred
• Strong organizational and time management skills
• Effective communication and interpersonal abilities
• Ability to work in a structured, fast-paced inpatient environment
• Basic computer and documentation proficiency
• Candidates selected for hire must successfully pass a criminal background check, drug screening, and any required state‑mandated clearances. Employment is contingent upon meeting all screening requirements in accordance with federal, state, and accrediting‑body regulations.

Responsibilities

• Coordinate aftercare planning to ensure patients have access to appropriate next levels of care
• Schedule appointments, meetings, and follow-up services for patients
• Arrange halfway house interviews and placements as part of discharge planning
• Communicate with referral sources, families, and external agencies regarding patient progress (as appropriate)
• Obtain and maintain necessary documentation, releases, and records
• Support patients in navigating resources to promote successful recovery outcomes
• Maintain accurate records and documentation related to case coordination activities
• Collaborate with clinical, medical, and operational teams to support patient care
• Perform other duties as assigned
