About The Position

Apex Companion Care, part of LHC Group and the Optum family of businesses, is hiring PRN Caregivers for multiple shifts in Jacksonville and surrounding areas. This home-based role supports individuals so they can remain in their own homes by providing personal care, help with daily living activities, and assistance with household functions. Caregivers help maintain a safe environment, monitor and document client conditions, and respond to client needs. Requirements

Current driver’s license and vehicle insurance, access to a dependable vehicle, or access to public transportation.
Ability to function in any home situation regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
Florida-specific: Successful completion of at least 40 hours of training in areas required by the Agency for Health Care Administration, or successful completion of the home health aide competency test (Form AHCA 3110-1007).
Candidates must pass a drug test before beginning employment.

Nice To Haves

Current CPR certification, or ability to complete CPR certification within 90 days of hire.
At least 6 months of home care experience.
Ability to work flexible hours as required to meet identified clients’ needs.

Responsibilities

Provide personal care and assist with daily living activities, including bathing, grooming, dressing, ambulation, and medication reminders.
Support household tasks and meal preparation.
Accompany clients to appointments or errands as needed.
Monitor and document client condition, vital signs, and incidents.
Maintain confidentiality and use the EVV system.
Ensure a safe environment and operate medical equipment properly.
Respond promptly to client needs.
