About The Position

Milestone is seeking compassionate Care Partners to support residents with daily living needs in its Assisted Living and Memory Care communities. The role provides personal care, supports meals and dining, and communicates important changes in residents’ conditions to the Health & Wellness team. Care Partners help ensure residents receive safe, high-quality, individualized care each day. The position may involve occasional weekend, evening, or night shifts for coverage and occasional travel.

Requirements

• High school diploma or equivalent.
• At least one year of caregiving or similar experience.
• Strong interpersonal, verbal, and written communication skills.
• Ability to communicate clearly in English and complete documentation accurately.
• Strong organizational skills, reliability, and attention to detail.
• Ability to work under pressure, prioritize tasks, and show initiative.
• Compassion for and desire to work with older adults.
• Ability to meet health requirements, including TB testing.
• Ability to pass required fingerprinting, background checks, and screenings.
• Certifications as required by state.

Nice To Haves

• Familiarity with Microsoft Word and Excel.

Responsibilities

• Follow each resident’s service plan in a friendly, respectful, and supportive manner.
• Document changes in resident condition according to company policy and state requirements.
• Support Continuous Quality Improvement by identifying issues and communicating them to leadership.
• Follow company policies and regulations, including HIPAA, OSHA, and infection control guidelines.
• Promote a safe working environment and immediately report unsafe actions.
• Report suspected elder maltreatment or abuse according to policy.
• Communicate effectively with fellow Care Partners about resident needs.
• Assist with meal service, including serving, clearing, and maintaining dining room cleanliness.
• Foster teamwork, respect, and positive interactions with residents, families, and coworkers.
• Perform other duties as assigned.
