About The Position

The Care Navigator plays a crucial role in providing a positive, welcoming experience for members and their families. The role guides members from the time they arrive until they leave the center, helping ensure a seamless visit and that their care needs are met. The ideal candidate has a strong customer-service orientation, is passionate about senior care, and is willing to go the extra mile to create a positive member experience.

Requirements

• Excellent customer-service skills and a positive, welcoming demeanor.
• Passion for providing a quality experience for senior members.
• Ability to work effectively with diverse populations, including staff, providers, members, family members, insurance carriers, vendors, and the public.
• Working knowledge of medical terminology, insurance, and/or electronic medical record systems.
• Strong organization and attention to detail.
• Strong written and verbal communication skills.
• Proficiency with PC skills, including Microsoft Office Suite.
• Ability to maintain professionalism and flexibility in a changing work environment.
• At least one year of work experience in a clinical setting or similar.

Nice To Haves

• Associate degree preferred, or equivalent experience.
• Experience working with EMR systems is a plus.
• Fluency in Spanish or other languages spoken by people in the communities served, where necessary.

Responsibilities

• Welcome members upon arrival and help them feel safe and at home while guiding them through check-in.
• Update member information in the electronic medical records system.
• Schedule appointments and coordinate referrals for a multidisciplinary care team.
• Assist members with paperwork through electronic kiosks as needed.
• Request medical records and upload documentation to the electronic medical records system.
• Answer questions from prospective and established members and their adult children.
• Answer incoming calls, take detailed messages, and convey relevant information to the appropriate person.
• Assist with center events as needed.
