About The Position
Reserve Care (formerly Hospice of the Western Reserve) is seeking a Care Navigation Nurse LPN to support referral intake, care coordination, and initial clinical triage. The role ensures timely, accurate, and efficient access to hospice services, emphasizing one-call resolution and seamless referral progression. Reserve Care operates with a mission to relieve suffering, enhance comfort, promote quality of life, foster choice in end-of-life care, and support effective grieving, all while celebrating individual worth.
Requirements
- Licensed Practical Nurse (LPN) in the state of Ohio
- Excellent verbal and written communication skills
- Ability to communicate calmly and compassionately with patients and families in distress
- Strong teamwork and collaboration skills
- Comfortable using multiple technologies, including Electronic Medical Records (EMR), phone systems, and referral and intake platforms
- Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
- Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
- Provide an active driver’s license
- Ability to provide proof of eligibility to work in the United States.
- High level of integrity, ethics and professionalism.

Responsibilities
- Retrieve, monitor, and process referrals for hospice services and Western Reserve Navigator potential patients from multiple sources, including CarePort, electronic fax inbox, phone referrals, and other designated referral sources.
- Learn and utilize the dispatch process to schedule appointments for prospective patients.
- Follow up on referrals that do not progress to dispatch in a timely manner.
- Perform direct admits to inpatient care centers as needed.
- Take clinical calls from patients and caregivers, attempt resolution, and escalate appropriately.
- Notify primary care teams of any clinical issues addressed.
- Participate in quality improvement and process improvement initiatives.
- Perform other duties as assigned.
