• Possess insured vehicle to travel to and from clients homes
• Ability to deal with others in a diplomatic, tactful and confidential manner
• Ability to maintain composure under stress in dealing with residents, staff, and families
• Flu vaccine required during flu season

Responsibilities

• Assessing and assisting with clients needs
• Coordinating care in a warm and compassionate environment
• Follow through and assist clients with their plan of care
• Coordinate care services ensuring that those we serve receive the highest quality of service in a caring and compassionate atmosphere
