About The Position

Senior Care Options by Parker is seeking a Care Manager to provide care management services to clients in Monmouth County. The role includes assessing clients’ needs, assisting them with those needs, following through on their plans of care, and coordinating care services. The Care Manager helps ensure clients receive high-quality service in a warm, compassionate environment. Services are provided to individuals in their homes and communities.

This part-time position is approximately 8 hours per week, with a Monday–Friday schedule between 9:00 a.m. and 5:00 p.m. The schedule is flexible, with the possibility of additional hours. The role involves travel to clients’ homes using an insured personal vehicle.

Requirements

• Bachelor’s degree required.
• Ability to read, write, understand, and communicate effectively in English.
• Valid driver’s license in good standing.
• Insured vehicle available for travel to and from clients’ homes.
• Ability to deal with others diplomatically, tactfully, and confidentially.
• Ability to maintain composure under stress and when dealing with residents, staff, and families.
• Flu vaccine required during flu season.
