About The Position

The Care Manager assists residents with all aspects of daily life as indicated in their care plans, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. The role provides care such as bathing, dressing, toileting, and feeding while maintaining residents’ safety, comfort, dignity, and confidentiality. The position reports to the Director of Wellness.

Requirements

• Certified Nursing Assistant (CNA) or Certified Home Health Aid (CHHA).
• Ability to perform intermittent lifting and support of residents, lift or carry up to 50 pounds, and use proper body mechanics.

• Knowledge and experience working with the elderly population.
• Dementia experience.

Responsibilities

• Provide resident care, including bathing, dressing, toileting, and feeding, as outlined in each resident’s care plan.
• Maintain the safety, comfort, dignity, and confidentiality of all residents.
• Escort residents to meals and activities.
• Ensure assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts.
• Document all services provided to assigned residents during the shift.
• Immediately report any change in a resident’s condition to a supervisor.
• Assist residents with food service, housekeeping, laundry, behavior management, socialization, and activities as indicated in their care plans.
