About The Position

Monarch Communities is seeking compassionate and dedicated Care Managers to assist residents with aspects of daily living according to their care plans. Duties may include personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. The position is led by the community's Director of Health and Wellness and serves residents in Assisted Living and/or Memory Care. This is a per-diem position paying $20.00–$22.00 per hour.

Requirements

High school diploma or GED.
At least 1 year of resident care experience.
Certified Nursing Assistant or Certified Home Health Aid.

Nice To Haves

Knowledge and experience working with the elderly population.
Dementia experience preferred.

Responsibilities

Assist residents with self-administration of prescribed medication by cueing, prompting, or reminding them, and maintain related medical records under RN and/or LPN supervision.
Provide care such as bathing, dressing, toileting, and feeding as outlined in residents’ care plans.
Adhere to schedules and properly document attendance.
Maintain residents’ safety, comfort, dignity, and confidentiality.
Encourage residents to attend meals and activities and escort them when needed.
Ensure assigned residents are clean, neatly dressed, groomed, safe, and fed during assigned shifts.
Document services provided to assigned residents in the EHR during each shift.
Immediately report changes in a resident’s condition to a supervisor.
Use electronic documentation devices appropriately and keep them safe and secure within the community; document care and services efficiently during the shift.
Participate in scheduled in-service trainings.
Comply with applicable federal, state, and local regulations and community policies and procedures.
Provide basic housekeeping and/or laundry services when needed.
Obtain resident weights and vital signs and document them when requested by an RN.
Perform other duties as assigned by a supervisor, RN, and/or Executive Director.
