About The Position

The Care Manager assists residents with all aspects of daily living as indicated in each resident’s care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. The role reports to the Director of Wellness. Compensation is up to $21.00 per hour. The posting identifies the employment category as per diem and also describes the role as part-time and per diem.

Requirements

Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA).
Ability to perform the listed physical activities, including standing, walking, sitting, using fine and gross motor skills, reaching, balancing, stooping, kneeling, crouching, talking, hearing, and smelling.
Ability to lift or carry up to 50 pounds and to lift and support residents intermittently, using proper body mechanics.

Nice To Haves

Knowledge and experience working with the elderly population; dementia experience is preferred.

Responsibilities

Provide resident care, including bathing, dressing, toileting, and feeding, as outlined in care plans.
Maintain residents’ safety, comfort, dignity, and confidentiality.
Escort residents to meals and activities.
Ensure assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts.
Document all services provided to assigned residents during the shift.
Immediately report any change in a resident’s condition to a supervisor.
