About The Position

The Care Manager assists residents with all aspects of daily living as outlined in their care plans. Duties may include personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. This is an hourly, non-exempt position reporting to the Health & Wellness Director. The posting describes part-time and per diem schedules: part-time shifts from 3 p.m. to 11 p.m., and per diem weekend shifts from 7 a.m. to 3 p.m. and 3 p.m. to 11 p.m.

Requirements

• Certified Nursing Assistant (CNA) or Certified Home Health Aid (CHHA).
• Ability to perform the physical activities described, including lifting or carrying up to 50 pounds and intermittently lifting and supporting residents.
• Must use proper body mechanics.

Nice To Haves

• Knowledge and experience working with the elderly population.
• Dementia experience.

Responsibilities

• Provide resident care, including bathing, dressing, toileting, and feeding, as outlined in care plans.
• Maintain residents’ safety, comfort, dignity, and confidentiality.
• Escort residents to meals and activities.
• Ensure assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts.
• Document all services provided to assigned residents during the shift.
• Immediately report any change in a resident’s condition to a supervisor.
• Assist with daily living needs that may include food service, housekeeping, laundry, behavior management, socialization, and activities.
