About The Position

The Care Manager assists residents with all aspects of daily living as indicated in each resident’s care plan. Support includes personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. The role is a part-time, non-exempt, hourly position and reports to the Health & Wellness Director. Part-time shifts are 3 p.m.–11 p.m.; per-diem weekend shifts are 7 a.m.–3 p.m. and 3 p.m.–11 p.m. Compensation is $20.00–$22.00 per hour.

Requirements

Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA).
Knowledge and experience working with the elderly population.
Ability to perform the physical activities required, including standing, walking, sitting, using fine and gross motor skills, reaching, balancing, stooping, kneeling, crouching, talking, hearing, and smelling.
Ability to lift or carry up to 50 pounds and intermittently lift and support residents, using proper body mechanics.

Nice To Haves

Dementia experience is preferred.

Responsibilities

Provide resident care, including bathing, dressing, toileting, and feeding, as outlined in care plans.
Maintain residents’ safety, comfort, dignity, and confidentiality.
Escort residents to meals and activities.
Ensure assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts.
Document services provided to assigned residents during the shift.
Immediately report any change in a resident’s condition to a supervisor.
