About The Position

Monarch Communities is seeking compassionate and dedicated Care Managers to assist residents in all aspects of daily life as indicated in each resident’s care plan. The role is in Assisted Living and/or Memory Care and is led by the community’s Director of Health and Wellness. Care Managers support residents with personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. The listed hourly salary range is $19.00–$22.00, with 6 a.m.–2 p.m. and 2 p.m.–10 p.m. shifts available. The position is 32–37.5 hours per week. Requirements

Certified Nursing Assistant (CNA).
Knowledge and experience working with the elderly population; dementia experience is preferred.

Responsibilities

Provide resident care, including bathing, dressing, toileting, and feeding, as outlined in care plans.
Maintain residents’ safety, comfort, dignity, and confidentiality.
Escort residents to meals and activities.
Ensure assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts.
Document all services provided to assigned residents during the shift.
Immediately report any change in a resident’s condition to a supervisor.
