About The Position

The Care Manager assists residents with all aspects of daily living specified in their care plans, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. This is a part-time, non-exempt, hourly position, with per-diem availability. The role reports to the Health & Wellness Director. The listed hourly salary range is $21.00–$24.00. The Care Manager supports residents’ safety, comfort, dignity, and confidentiality; documents services provided during each shift; and promptly reports changes in residents’ conditions to a supervisor. The job involves frequent standing, walking, sitting, reaching, balancing, stooping, kneeling, and crouching, as well as intermittent lifting and supporting of residents. The associate must use proper body mechanics and may need to lift or carry up to 50 pounds.

Requirements

Certified Nursing Assistant (CNA) or Certified Home Health Aid (CHHA).
Ability to perform the physical duties of the job, including intermittent lifting and supporting of residents and lifting or carrying up to 50 pounds.
Use proper body mechanics.

Nice To Haves

Knowledge and experience working with the elderly population.
Dementia experience is preferred.

Responsibilities

Provide resident care, including bathing, dressing, toileting, and feeding, as outlined in each resident’s care plan.
Maintain residents’ safety, comfort, dignity, and confidentiality.
Escort residents to meals and activities.
Ensure assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts.
Document all services provided to assigned residents during the shift.
Immediately report any change in a resident’s condition to a supervisor.
Assist residents with daily living needs specified in their care plans, including food service, housekeeping, laundry, behavior management, socialization, and activities.
