Care Manager (CNA/CHHA)
About The Position
The Care Manager provides warm, engaging personal care services, support, and companionship to residents. The role assists residents with activities of daily living, monitors changes in residents’ daily condition and personal care needs, and helps keep each resident’s care plan updated. The Care Manager also encourages residents to participate in community programs and activities. The position is per diem at Monarch Communities/Brandywine Senior Living’s location at 41 Springfield Avenue, Summit, New Jersey. The listed shift is 7 a.m.–3 p.m. Employees must be available for at least four shifts per month, including at least one weekend shift and one holiday shift per month. The pay range is $19–$20 per hour, with final compensation based on experience and qualifications.
Requirements
- High school diploma or equivalent.
- Current CNA certification.
- Positive attitude and enjoyment of working with people.
- Availability for a minimum of four shifts per month, including at least one weekend shift and one holiday shift per month.
Responsibilities
- Provide warm, engaging personal care services, support, and companionship to residents.
- Engage and assist residents with activities of daily living (ADLs).
- Observe changes in residents’ daily condition and personal care needs and update their Resident Care Plan.
- Encourage residents to participate in community programs and activities.
