About The Position

Community Care is hiring a Care Manager to join its Family Care Program team. The role manages an assigned caseload of program members in collaboration with an RN Care Manager and supports elderly individuals and adults with physical and intellectual disabilities in living as independently as possible in the community. The position is based in Outagamie County and involves regular local travel using a personal vehicle. The Care Manager completes comprehensive member assessments, member-centered care plans, and functional screens; coordinates care and monitors service quality; and maintains documentation in accordance with established standards. This is a full-time weekday position, scheduled Monday through Friday, 8:00 AM to 4:30 PM.

Requirements

A bachelor’s degree in human services or a closely related area is required. A minimum of three years of experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human-services degree.
Eligibility for long-term care functional screener certification is required.
Regular local travel is required, using a personal vehicle; a valid driver’s license and proof of required insurance coverage are required.

Nice To Haves

One year of experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity is preferred.
