About The Position

Community Care is hiring a Care Manager/Case Manager for its Family Care Program team in Outagamie County. The role supports elderly individuals and adults with physical and intellectual disabilities in living as independently as possible in the community. The Care Manager collaborates with an RN Care Manager and manages an assigned caseload of program members. This is a full-time opportunity with a weekday schedule and regular local travel.

Requirements

• Bachelor’s degree in Human Services or a closely related area; a minimum of three years’ experience working with seniors or adults with physical and/or intellectual disabilities may substitute for a non-human-services degree.
• Eligibility for long-term care functional screener certification is required.
• Regular local travel requiring use of a personal vehicle, a valid driver’s license, and proof of required insurance coverage.

Nice To Haves

• One year of experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity is preferred.

Responsibilities

• Manage an assigned caseload of program members in collaboration with an RN Care Manager.
• Complete comprehensive member assessments, member-centered care plans, and functional screens.
• Provide ongoing care coordination and quality assurance monitoring of services.
• Maintain required documentation in accordance with established standards.
