About The Position
Community Care is hiring a Care Manager to join its Partnership Program team. The role manages an assigned caseload of program members in collaboration with an RN Care Manager and supports older adults and adults with physical and intellectual disabilities in living as independently as possible in the community. The position is based in Racine County and involves regular local travel using a personal vehicle. It is a full-time weekday role scheduled Monday through Friday, 8:00 AM to 4:30 PM, and offers a $5,000 sign-on bonus.
Requirements
- Bachelor’s degree in Human Services or a closely related area; a minimum of three years’ experience working with seniors or adults with physical and/or intellectual disabilities may substitute for a non-Human Services degree.
- Eligibility for long-term care functional screener certification.
- Regular local travel using a personal vehicle, with a valid driver’s license and proof of required insurance coverage.
Nice To Haves
- One year of experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity.
Responsibilities
- Manage an assigned caseload of program members in collaboration with an RN Care Manager.
- Complete comprehensive member assessments, member-centered care plans, and functional screens.
- Provide ongoing care coordination and quality assurance monitoring of services.
- Maintain required documentation in adherence to established standards.
