About The Position

Community Care is hiring a Care Manager to join its Family Care Program team. The role supports elderly individuals and adults with physical and intellectual disabilities in living as independently as possible in the community. The Care Manager manages an assigned caseload in collaboration with an RN Care Manager, completes assessments and care plans, coordinates services, monitors service quality, and maintains required documentation. The position is based in Racine County and involves regular local travel using a personal vehicle. This is a full-time weekday opportunity with a Monday–Friday schedule from 8:00 AM to 4:30 PM.

Requirements

• A bachelor’s degree in Human Services or a closely related area is required. Three years of experience working with seniors or adults with physical and/or intellectual disabilities may substitute for a non-human-services degree.
• Eligibility for long-term care functional screener certification is required.
• Regular local travel requiring use of a personal vehicle, a valid driver's license, and proof of required insurance coverage.

Nice To Haves

• One year of experience with older adults or people with physical and/or intellectual disabilities in a social service capacity is preferred.

Responsibilities

• Manage an assigned caseload of program members in collaboration with an RN Care Manager.
• Complete comprehensive member assessments, member-centered care plans, and functional screens.
• Provide ongoing care coordination and quality assurance monitoring of services.
• Maintain required documentation in adherence to established standards.
