Care Manager MSW - Rehab Unit
About The Position
The Inpatient Rehab Care Manager MSW works collaboratively on an interdisciplinary consulting team to support patients and families dealing with serious illness. The role provides a full range of social work services, including psychosocial assessment, treatment planning, therapeutic and crisis interventions, resource referrals, short-term counseling, grief support, advance care planning, and facilitation of family meetings in collaboration with the interdisciplinary team. The Care Manager MSW also takes a leadership role in facilitating patient goals, resource outcomes, and family meetings. This position is with Providence St. Mary Medical Center. Requirements
- Master's degree in Social Work or a related field from an accredited educational institution.
- Upon hire, Washington Clinical Independent Social Worker Associate License or Washington Clinical Independent Social Worker License.
Nice To Haves
- One year of recent inpatient clinical experience in an acute care facility.
- Case management experience.
- Health care utilization review, discharge planning, and/or social work experience.
- Psychiatric-based experience.
Responsibilities
