Care Manager MSW - Case Management
About The Position
The Inpatient Master of Social Work (MSW) Care Manager provides professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment. The role supports continuity of care for vulnerable patients by identifying resources to address social, financial, cognitive or behavioral, and legal barriers to accessing care. The position includes assessment and planning, care coordination, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy. This full-time role is based at Providence St. Joseph Hospital in Eureka, California, and works 10-hour day shifts.
Requirements
- Master's degree in social work from an accredited CSWE program.
- At least 1 year of social work experience in any healthcare setting.
- At least 1 year of experience in an acute care setting, or successful completion of the TIPs program or Case Management Orientation Program.
Nice To Haves
- Basic understanding of acute medical-surgical healthcare terminology.
- Prior case management experience.
