About The Position

Oak Plains Academy, a UHS company, is hiring a Case Manager (MSW) for its 98-bed residential treatment program for adolescents experiencing emotional and behavioral issues. The program provides individualized treatment through a professional team that includes psychiatrists, psychologists, social workers, therapists, nurses, and teachers. The rural campus is located about 45 minutes from Nashville, Tennessee, and includes a gymnasium, dining hall, academic building, dormitories, and wooded areas for outdoor recreation.

The Case Manager provides case management, admission support, utilization review, and services to residents, their families, and care providers. The role serves on the treatment team, supports the organization’s treatment program and philosophy, and helps ensure quality treatment for residents and families. Relevant knowledge includes case management, discharge planning, age-specific growth and development, crisis and behavior management, individual and family therapy theory and practice, behavioral health, and substance use.

Requirements

• Master’s degree in Social Work.
• Licensed as an LMSW, LPC, LCSW, or LMFT, or eligible for a licensure track in social work or a related field.
• One to three years of prior experience working with a psychiatric behavioral health population.
• Successful background check, fingerprinting, and employment verification.
• Valid driver’s license.

• Three to five years of experience working with a psychiatric behavioral health population is preferred.
