About The Position
Regions Hospital is hiring a Care Management Assistant to join our Care Management team. In this role, you will support safe, timely transitions of care for patients. You will partner with RN and Social Work Care Managers to coordinate referrals, post-acute and community resources, insurance authorizations, patient and family communication, documentation, and data collection. You will collaborate with the interdisciplinary care team to identify and escalate barriers that could delay a patient’s transition from the hospital.
Requirements
- Associate degree in healthcare, human services, business, or a related field, or an equivalent combination of education and relevant experience
- Previous experience in healthcare, patient-facing care, care coordination, or administrative support
- No license or certification required
Nice To Haves
- Bachelor’s degree
- 2+ years of healthcare experience
- Experience in hospital care management, care coordination, discharge planning support, health unit coordination, medical assisting, patient care, or a related setting
- CNA, HUC, or other relevant healthcare training or certification
Responsibilities
