Care Coordinator - Davita IKC - Southern Atlanta
About The Position
DaVita Integrated Kidney Care is hiring a Care Coordinator to help patients with complex chronic conditions navigate their care. The coordinator works with Nurse Practitioners and RN Case Managers to coordinate care, connect patients with resources, and support their progress through the continuum of care. The position follows a full-time Monday–Friday schedule. It requires commuting to DaVita clinics 3–4 times per week, with the remaining work time from home. Daily start time is between 7:00 and 8:00 AM, with flexibility as needed to meet patient and clinic needs. Reliable transportation is required, and mileage is reimbursed. When working from home, the teammate must have a quiet, private workspace and high-speed internet; DaVita provides the full IT setup.
Requirements
- High school diploma or GED.
- At least 2 years of experience in a professional office environment.
- Ability to handle confidential information with discretion and sound judgment.
- Strong verbal communication skills.
- Proficiency in Microsoft Office, Excel, and Outlook.
- Commitment to service excellence and patient satisfaction.
- Ability to work autonomously while collaborating effectively with the team.
- Reliable transportation for required clinic travel.
- For work from home, a quiet, private space and high-speed internet.
Nice To Haves
- Prior experience with Electronic Medical Records.
- 1–3 years of clinical medical office experience.
- CNA or MA experience.
- Experience coordinating care, such as referrals or care management.
- Familiarity navigating insurance payers.
- Experience in direct patient care or support.
- Prior medical office environment experience.
Responsibilities
- Coordinate patient care by ensuring access to eligible resources and programs and assisting with referrals and provider appointments.
- Make outbound calls and respond to inbound calls from patients, providers, and Village resources.
- Complete non-clinical screenings and pathways under the Manager’s guidance, escalating to the Integrated Care Nurse as needed.
- Complete assigned tasks to help the clinical team move patients through the continuum of care.
- Identify eligible member resources and collaborate with Integrated Care Nurses, dialysis clinics, health plans, and other stakeholders to help members enroll in programs.
- Communicate directly with patients in a timely and professional manner.
- Maintain accurate, complete, and confidential documentation.
- Demonstrate effective two-way oral and written communication.
