About The Position

DaVita Integrated Kidney Care is hiring a Care Coordinator to help patients with complex chronic conditions navigate their care. The coordinator works closely with Nurse Practitioners and RN Case Managers to coordinate care, facilitate access to resources, and support patients through the continuum of care. This is a full-time, Monday–Friday hybrid role. The employee commutes to DaVita clinics as required, typically 3–4 times per week, and works the remaining time from home. Daily start time is between 7:00 and 8:00 AM, with flexibility required to meet patient and clinic needs. Reliable transportation is required, and mileage is reimbursed. At-home work requires a quiet, private space with high-speed internet; DaVita provides the full IT setup.

Requirements

High school diploma or GED.
At least 2 years of experience in a professional office environment.
Ability to handle confidential information with discretion and sound judgment.
Strong verbal communication skills.
Proficiency in Microsoft Office, Excel, and Outlook.
Commitment to service excellence and patient satisfaction.
Ability to work autonomously while collaborating effectively with the team.
Reliable transportation for required clinic commutes.
When working from home, a quiet, private space with high-speed internet.

Nice To Haves

Prior experience with Electronic Medical Records.
1–3 years of clinical medical office experience.
CNA or MA experience.
Experience coordinating care, including referrals or care management.
Familiarity navigating insurance payers.
Experience in direct patient care or support.
Prior medical office environment experience.

Responsibilities

Coordinate care by helping patients access eligible resources and programs and assisting with referrals and provider appointments.
Make outbound calls and respond to inbound calls from patients, providers, and Village resources.
Complete non-clinical screenings and pathways under the Manager’s guidance, escalating to the Integrated Care Nurse as needed.
Complete tasks as directed to help the clinical team move patients through the continuum of care.
Identify eligible resources and collaborate with Integrated Care Nurses, dialysis clinics, health plans, and other stakeholders to help members enroll in programs.
Communicate with patients in a timely and professional manner.
Maintain accurate, complete, and confidential documentation.
Demonstrate exceptional two-way oral and written communication.
Act in alignment with DaVita’s mission and values to deliver high-quality patient care.
