DaVita Integrated Kidney Care is hiring a Care Coordinator to support patients with complex chronic conditions. The Care Coordinator works closely with Nurse Practitioners and RN Case Managers to coordinate care, facilitate access to resources, and help keep patients moving through the care continuum. The role involves working in clinics and partner practices as needed, typically 3–5 days, and commuting to multiple DaVita clinics and/or partner practices in the listed service area. Mileage is reimbursed, and reliable personal transportation is required. The position is full-time, Monday through Friday, with a daily start time between 7:00 and 8:00 AM. Flexibility is required to meet patient and clinic needs. When working from home, the teammate must have a quiet, private workspace and high-speed internet. The Care Coordinator supports patient access to eligible resources and programs, assists with referrals and provider appointments, and performs non-clinical screenings and pathways under a Manager’s guidance. The role includes frequent communication with patients, providers, Village resources, clinical teams, dialysis clinics, health plans, and other stakeholders, along with maintaining accurate, complete, and confidential documentation. The position calls for strong two-way communication, service excellence, sound judgment, independent work, and effective collaboration. DaVita describes its workplace as a mission-driven, collaborative community focused on belonging and high-quality patient care. The company provides a competitive rewards package, with benefits eligibility beginning on the first day of the month following or coinciding with one month of continuous employment. The role is open for a minimum of three days. The posted wage range is $19.29–$22.50 per hour, with compensation dependent on qualifications, skills, competencies, and experience.
