As a Care Coordinator, you’ll play a key role in helping individuals get started with services, stay on track with their care, and connect to essential resources. You’ll also coordinate intakes, schedule appointments, verify insurance, assist with telehealth access, and ensure accurate documentation in the electronic health record (EHR). You’ll also collaborate with case managers and clinic assistances to link clients to community resources, track prescriptions, and promote a smooth, trauma-informed care experience. Provide Psychosocial rehab groups under Medicaid requirements.
